Care Plan: Culture Change
(CP_Cult_Chg)

It is a fact that the survey guidance has been rewritten to provide for a "person centered" approach in activities. It is the main objective for surveyors to pull a sample of Care Plans from the facility and make sure that they provide correct and person centered language for that sample. This workshop is designed to make sure you, the Activity Director, are ready. It includes a very in depth look at creating a Person Centered Care Plan for each and every one of your residents that will in turn help you to provide the best quality of life possible.

Workshop Objectives:

  • Replace outdated documentation habits with new up to date standards.
  • Revise current understanding on the basics of care planning with the addition of new techniques.
  • Understand the most common terms to use while documenting for better communication between the facility, administrator, and surveyor.
  • Understand the shift of importance to non-clinical services provided by the facility.
  • Understand the differences between The Medical Model of Care and the new Person Centered Model of Care.
  • Ability to research, extract and compile factual information from which to build your plan of care from.
  • Ability to comprehend and employ the proper plan of care from the information gathered and personal assessment.

  • Before We Get Started, What is Culture Change?
  • Evolution of Change
