2013-03-18 05:55:46 PM cchase how are you tonight
2013-03-18 05:55:58 PM dtkimble tired but that is typical
2013-03-18 05:56:01 PM dtkimble you?

2013-03-18 05:58:52 PM kmcooley Hello
2013-03-18 05:59:04 PM dtkimble Dawn Kimble
2013-03-18 05:59:12 PM sarahdecollibus Hi Hows everyone doing ?
2013-03-18 05:59:28 PM dtkimble crazy day today! I am over it though
2013-03-18 05:59:40 PM cchase well dawn - I am the adult day & Alzheimer's ADN consultant
2013-03-18 05:59:56 PM dtkimble Nice to meet you!
2013-03-18 06:00:01 PM cchase same here
2013-03-18 06:00:08 PM kmcooley Is the Alheimer's specialty chat tonight?
2013-03-18 06:00:11 PM dtkimble thank you.
2013-03-18 06:00:19 PM cchase how is this class going for you?
2013-03-18 06:00:25 PM sarahdecollibus We made easter baskets and sold them to raise money for spring events
2013-03-18 06:00:29 PM kmcooley Alzheimers...
2013-03-18 06:00:50 PM cchase well - we can discuss that topic as well
2013-03-18 06:01:01 PM dtkimble Week 6 assignments were almost a repeat of week 4's in a way....
2013-03-18 06:01:01 PM kmcooley O.k.
2013-03-18 06:01:07 PM cchase what would you like to know about Alzheimer's?
2013-03-18 06:01:37 PM dtkimble how can 2 Act. Aides manage 5 behaviors from 3-4pm when they are not CNA's...
2013-03-18 06:01:41 PM kmcooley Mostly more ideas for "engaging" residents when they are at loose ends
2013-03-18 06:02:13 PM cchase should they be cna's??
2013-03-18 06:02:26 PM dtkimble We have 10am and 2pm activities planned and from 3-4 we have 5 residents who are physically active with dementia.
2013-03-18 06:02:49 PM kmcooley 5 is a lot...
2013-03-18 06:02:53 PM dtkimble there are cna's providing care but the 2 in activities are activity aides
2013-03-18 06:03:14 PM dtkimble they are sundowners who begin the energy cycle before dinner
2013-03-18 06:03:38 PM cchase our activity aides are certified in the care of Alzheimer's
2013-03-18 06:03:46 PM cchase I personally train them
2013-03-18 06:03:55 PM dtkimble they try to do a ball play or parachute at 2pm to tire them out
2013-03-18 06:04:12 PM cchase they are certified via my class
2013-03-18 06:04:38 PM cchase that's not such a bad idea
2013-03-18 06:04:43 PM dtkimble My girls are very good with the residents....I have not seen better....it is just difficult when one takes off, one gets up and the chair alarm goes off
2013-03-18 06:05:05 PM cchase be careful of getting them overwhelmed
2013-03-18 06:05:07 PM dtkimble it is hard to engage 5 behaviors with 2 staff....
2013-03-18 06:05:23 PM kmcooley our chair alarms go off a lot!
2013-03-18 06:05:57 PM cchase you have ratio of 1 to 2 or so
2013-03-18 06:05:58 PM dtkimble I have about 3 activity staff that I feel are very good with the dementia residents....sometimes I am able to send down some help...have one person do nails....another will sing...
2013-03-18 06:06:01 PM cchase not too bad
2013-03-18 06:06:23 PM cchase good strategy
2013-03-18 06:06:29 PM kmcooley Ours ofter dose off and on because they are often up and down during the night
2013-03-18 06:06:37 PM dtkimble it is just the one hour of the busiest residents
2013-03-18 06:06:50 PM cchase sometimes a group things is best at this time -- like music
2013-03-18 06:06:57 PM kmcooley Singing is a good idea...will try
2013-03-18 06:07:04 PM dtkimble that's what we do from 2-3...
2013-03-18 06:07:23 PM cchase you should leave that for a latter time
2013-03-18 06:07:38 PM sarahdecollibus sometimes when they are agitated they will sort things
2013-03-18 06:07:56 PM cchase you need to maximize the engagment vs the no of residents
2013-03-18 06:08:05 PM dtkimble we have been trying so many different things...there are 48 residents in the dementia unit
2013-03-18 06:08:07 PM cchase sorting is good
2013-03-18 06:08:29 PM cchase can you switch things up?
2013-03-18 06:08:35 PM sanrob enter the chat room
2013-03-18 06:08:39 PM dtkimble one gal actually said..."am I your maid?" it is almost comical..
2013-03-18 06:08:42 PM cchase Hello
2013-03-18 06:08:46 PM kmcooley we have a ball of yarn in a plastic coffee can with a little hole in the top for the string to go through...the resident winds the yarn into a ball...sometimes it works
2013-03-18 06:08:53 PM cchase really?? funny
2013-03-18 06:09:07 PM sanrob Hi everyone!
2013-03-18 06:09:12 PM kmcooley 48!!
2013-03-18 06:09:12 PM cchase good thinking with the yarn
2013-03-18 06:09:17 PM cchase helllo
2013-03-18 06:09:29 PM sanrob What is the topic?
2013-03-18 06:09:32 PM cchase 48 dementia at one time is huge
2013-03-18 06:09:49 PM cchase not sure anyone can deal with that
2013-03-18 06:10:08 PM dtkimble the cna's in the cottages are supposed to bring 2 residents from each of the four cottages...
2013-03-18 06:10:13 PM dtkimble but, they don't.
2013-03-18 06:10:15 PM cchase would break them down in groups
2013-03-18 06:10:28 PM cchase why don't they??
2013-03-18 06:10:46 PM kmcooley We had two old hand mixers with the cords cut off ...two of the dementia residents really enjoyed taking them apart...any ideas for safe ways of doing similar things?

2013-03-18 06:11:01 PM dtkimble they feel activities are for the activity dept. we have had this issue many times over
2013-03-18 06:11:04 PM cchase hello
2013-03-18 06:11:15 PM dtkimble they claim they are too busy providing personal care
2013-03-18 06:11:15 PM cchase this is not a new thing
2013-03-18 06:11:32 PM sanrob Same old song! To busy.
2013-03-18 06:11:47 PM cchase the cross over bwt disciplines is cloudy at time
2013-03-18 06:12:11 PM dtkimble We have a new unit nurse. She has been very good at listening. I find the staff takes "ownership" of their cottage residents.
2013-03-18 06:12:26 PM dtkimble sometimes this feels absolutely wrong.
2013-03-18 06:12:26 PM cchase there job is to ready the residents for other things like activities
2013-03-18 06:12:39 PM dtkimble instead, they put them to bed
2013-03-18 06:12:48 PM khurley2013 left the chat room
2013-03-18 06:12:59 PM kmcooley ...yes...happens here at times too
2013-03-18 06:13:07 PM cchase easier I suppose to put them to bed
2013-03-18 06:13:10 PM dtkimble I made it a point to tell the nurse that several residents will need to attend bingo.
2013-03-18 06:13:21 PM cchase but your DON should be on it
2013-03-18 06:13:24 PM dtkimble please give them a morning nap if they are ready.
2013-03-18 06:13:50 PM dtkimble he used to be the floor nurse. He is aware of the workings of that staff. He is now the DON
2013-03-18 06:13:51 PM cchase stimulation is so important for this group
2013-03-18 06:14:26 PM sanrob We have a protocol, talk to the nurse and let her know you will document in the chart to care plan for times these residents should get up. That If you don't see results go to the suoervisor and on up if necessary. Its all about the well being of the resident.
2013-03-18 06:14:26 PM dtkimble yes it is. We have a fellow who is doing very well socializing at bingo now. For months he was just sitting in the cottage
2013-03-18 06:14:45 PM cchase our job is not to make our lives easier but to help these people keep what they are so quickly losing
2013-03-18 06:15:15 PM sanrob QUALITY of LIFE! Theirs!
2013-03-18 06:15:21 PM cchase right
2013-03-18 06:15:27 PM dtkimble I will do this. Because I am new to the director position, I feel that some look to me as trouble....these are things that have
2013-03-18 06:15:34 PM dtkimble needed addressing for some time!
2013-03-18 06:15:42 PM dtkimble because it IS about the quality of life
2013-03-18 06:15:54 PM cchase you can do this in a diplimatic way
2013-03-18 06:16:04 PM kmcooley Especially, the res. with dementia need to be brought down or they'll forget
2013-03-18 06:16:15 PM cchase so sincere concern for those residents
2013-03-18 06:16:19 PM dtkimble I am probably one of the most diplomatic in the facility
2013-03-18 06:16:24 PM cchase good
2013-03-18 06:16:33 PM cchase then you will succeed
2013-03-18 06:16:41 PM dtkimble I hope to.

2013-03-18 06:16:50 PM cchase I think so
2013-03-18 06:16:54 PM cchase Hello there
2013-03-18 06:17:03 PM sanrob I always use tact, kindness and finess with cna's and nurses. I try to make it seem as if I respect what they are able to do to make this happen. Talk up how good they are at their job. Appreciate them and they will bend over backwards to help you more.
2013-03-18 06:17:10 PM cchase it's all about those residents
2013-03-18 06:17:10 PM angel4aml18 hello all, sorry I'm late....
2013-03-18 06:17:15 PM dtkimble I hav very few problems in the other areas of the facility.
2013-03-18 06:17:25 PM cchase no worries
2013-03-18 06:17:53 PM cchase sometimes it's hard to get everyone thinking that they are on the same team
2013-03-18 06:17:55 PM dtkimble Is it typical that the caregivers in the dementia unit to be a bit difficult?
2013-03-18 06:18:14 PM cchase caregivers are difficult??
2013-03-18 06:18:24 PM dtkimble rough.
2013-03-18 06:18:27 PM cchase do not understand that
2013-03-18 06:18:35 PM dtkimble not with the residents but with each other...and staff.
2013-03-18 06:18:37 PM cchase rough with whom
2013-03-18 06:18:52 PM cchase it's not about them
2013-03-18 06:19:03 PM cchase it's about those they serve
2013-03-18 06:19:09 PM sanrob As activity directors and assistants we have to empower the ones we depend on who are not in our department. It may take some time but eventually most will be willing.
2013-03-18 06:19:13 PM dtkimble the aides work in the same cottages day in and day out. they know the residents well but can be stifling
2013-03-18 06:19:13 PM kmcooley maybe it is like "combat fatigue"...

2013-03-18 06:19:43 PM cchase hello angel
2013-03-18 06:19:48 PM dtkimble that is actually what I thought....the combat fatigue.
2013-03-18 06:19:55 PM angel4aml18 you just have to kill them with kindness...hello cchase :)
2013-03-18 06:19:58 PM cchase yes - after awhile they get numb
2013-03-18 06:20:06 PM cchase hi there
2013-03-18 06:20:18 PM dtkimble My activity aides do a terrific job but they do look worn out sometimes. I will plan on spending more time down there
2013-03-18 06:20:30 PM dtkimble when I get caught up...(if ever)
2013-03-18 06:20:43 PM kmcooley what's "caught up"?
2013-03-18 06:20:47 PM cchase well - they are over burnt
2013-03-18 06:20:47 PM sanrob I suggest we keep our ways of dealing with the rest of the staff with fresh new ideas, anyone have any?
2013-03-18 06:20:56 PM cchase ha ha caught up
2013-03-18 06:21:04 PM dtkimble really.
2013-03-18 06:21:15 PM dtkimble teams, meetings more meetings...
2013-03-18 06:21:15 PM cchase remember that each day is a new day for them
2013-03-18 06:21:25 PM cchase and so it must be the same for you
2013-03-18 06:21:34 PM dtkimble good though
2013-03-18 06:21:36 PM dtkimble t
2013-03-18 06:21:43 PM angel4aml18 we do staff appreciation projects 1x/month to let them know they are appreciated
2013-03-18 06:21:53 PM cchase they do not hold grudges
2013-03-18 06:22:01 PM cchase live only in the moment
2013-03-18 06:22:02 PM dtkimble that is important
2013-03-18 06:22:02 PM sanrob CNA's have a tough job. Any acts of recognition and kindness helps the medicine go down, so to speak.
2013-03-18 06:22:21 PM dtkimble CNA
2013-03-18 06:22:28 PM cchase that goes all the way around
2013-03-18 06:22:33 PM dtkimble 's do have a tough job
2013-03-18 06:22:38 PM cchase from cna to Activity staff
2013-03-18 06:22:56 PM cchase activities have a tough job too
2013-03-18 06:23:04 PM dtkimble we have a gentleman who is on a steady health decline
2013-03-18 06:23:14 PM sanrob True! But we get to do the fun stuff!
2013-03-18 06:23:18 PM dtkimble it is so sad. He doesn't even open his eyes at breakfast.
2013-03-18 06:23:22 PM kmcooley I made a project folder for a former engineer...it had technical information on a project he'd worked on ( a public building) ...I set the manilla folder (with pages stapled in) on a table nearby and walked away. He of course picked it up and was engrossed...for awhile...need to make a new one
2013-03-18 06:23:30 PM angel4aml18 I wouldn't want to do their job for the pay they make, it is hard but they chose the job so if they don't like it then they should find something they are happy doing. all it does it bring everyone else around them doew esp. the residents
2013-03-18 06:23:45 PM cchase keeping cognitive engagement when all the functions are declining is pretty challenging
2013-03-18 06:24:09 PM dtkimble we involve him in sensory, hand massage, music. Nothing seems to stir him
