2013-06-26 06:03:49 PM cchase hello there
2013-06-26 06:04:12 PM cchase how are you doing tonight?
2013-06-26 06:04:51 PM smmbruin10 hi. i'm well. just trying to figure out why chat room for MEPAPII isn't working. what class are you in?
2013-06-26 06:05:52 PM cchase this is the Alzheimer's chat room - I am the ADN consultant for Alzheimer's and Adult Day. I am a former student too
2013-06-26 06:07:11 PM cchase what do you want to learn about Alzheimer's?
2013-06-26 06:08:12 PM smmbruin10 Oh, sorry! Hi. Yeah, so I think that the link for the chat rooms in the MEPAPII-OWN PACE course isn't working properly. I've been communicating with one other student in the own pace class and we haven't been able to find others. I only found you by logging in from my old MEPAP1 course link.
2013-06-26 06:09:40 PM cchase did you see the schedule on the main page - I think it's on the right side of the computer screen? it shows what's upcoming and this chat was posted there to remind students
2013-06-26 06:10:08 PM smmbruin10 no, nothing has been posted in the Own Pace course
2013-06-26 06:10:12 PM cchase you get extra credit for attending this specialty chat
2013-06-26 06:10:42 PM smmbruin10 we don't even have a chat schedule with dates. just a note from Kathy saying that we should try to sign in every Thursday.
2013-06-26 06:10:51 PM cchase I will have to check with Kathy on that! I imagine that own pace should see it too
2013-06-26 06:11:18 PM smmbruin10 I think we should too! haha I'm gonna jumo over and let my classmate know what's going on. brb
2013-06-26 06:11:32 PM cchase well - looks like you are earing extra credit tonight :)

2013-06-26 06:15:17 PM dianajacoby I'm looking for the specialty chat room. Is this it for tonight?
2013-06-26 06:15:26 PM cchase yes it is
2013-06-26 06:15:47 PM dianajacoby Great, thanks! Is this for ADHC or Alzheimer's?
2013-06-26 06:16:22 PM cchase Tonight is Alzheimer's - I will do one for ADH On July 3rd

2013-06-26 06:16:40 PM cchase hi again!
2013-06-26 06:16:47 PM smmbruin10 hello again.
2013-06-26 06:17:01 PM cchase did you find anyone else out there?
2013-06-26 06:17:05 PM dianajacoby Ok. I'm actually interested and working with both populations, and I"m still getting the hang of this format of discussion.
2013-06-26 06:17:32 PM cchase it does take getting used to for sure. . . :(
2013-06-26 06:17:41 PM cchase ({)
2013-06-26 06:17:52 PM smmbruin10 my classmate in MEPAP2-Own Pace is attempting to sign in now. Her name is Elisa Klemetsen.
2013-06-26 06:18:03 PM cchase great!
2013-06-26 06:18:14 PM dianajacoby So is there a topic of discussion generated, or how does this work?
2013-06-26 06:18:56 PM smmbruin10 we definitely have a faulty link in our course that directs us to the old chat format.
2013-06-26 06:19:03 PM cchase do you all work in special Alz units or are they in around difference locations of your faciliity/
2013-06-26 06:19:30 PM cchase really? should email kathy about that
2013-06-26 06:19:36 PM dianajacoby I have an early-moderate social day program in Eugene, OR as part of a non-profit.
2013-06-26 06:19:58 PM cchase I used to be able to see who is in the chat room but do not see that anymore
2013-06-26 06:20:17 PM cchase tell me about your social day program
2013-06-26 06:20:22 PM dianajacoby My program is in the process of being acquired by a memory care facility that will be expanding services to include an ADHC.
2013-06-26 06:21:04 PM cchase great- try to attend the ADH specialty chat on Jul 3rd - I will moderate that class
2013-06-26 06:21:09 PM smmbruin10 yeah, i will....I worked in Assisted Living/independent living facility in our memory care department. I worked with a program called the Circle of Friends. our residents had MCI or early dementia. and we had a secured unit for advanced alz residents
2013-06-26 06:21:41 PM cchase I have worked in such a place before. .
2013-06-26 06:22:02 PM cchase okay - so tell me what you would like to learn about alzheimer's
2013-06-26 06:22:03 PM dianajacoby The program has a cognitive wellness component to it as well as a social program. It operates from 9a-4p weekdays. We have a fitness instructor daily, an art instructor weekly, we go on outings, and participate in community service projets.
2013-06-26 06:22:48 PM cchase is it a medical model?
2013-06-26 06:22:53 PM dianajacoby no, social
2013-06-26 06:23:00 PM cchase got it
2013-06-26 06:23:29 PM smmbruin10 I actually specialized in Alz care, so I hope I know more than the average! My goal is to open and run my own adult day facility one day
2013-06-26 06:23:47 PM dianajacoby My challenge is only coming up as the program is expanded to licensed adhc.
2013-06-26 06:24:07 PM dianajacoby The MEPAP has been helpful learning the regs, etc.
2013-06-26 06:24:43 PM cchase what do you both want to learn about Alzheimer's - what training have you both had in the field
2013-06-26 06:25:31 PM dianajacoby I have my M.S. in Gerontology and have been working with this population for several years.
2013-06-26 06:25:34 PM smmbruin10 i took a handful of courses in college, minored in Gerontology. We had to go through a 10 hour dementia training at work
2013-06-26 06:25:45 PM kdillon enter the chat room

2013-06-26 06:26:49 PM cchase what makes you desire the ADC certification

2013-06-26 06:27:10 PM dianajacoby We have 4 support groups for caregivers each week, and one for early-moderate memory loss. I'll be applying for the ACC.
2013-06-26 06:27:40 PM eklemetsen hello. is anyone there
2013-06-26 06:27:41 PM smmbruin10 expanding job opportunities! I didn't get a job recently because I wasn't certified.
2013-06-26 06:27:48 PM cchase hello there
2013-06-26 06:27:50 PM smmbruin10 you found us, Elisa!
2013-06-26 06:28:10 PM eklemetsen but I can't find Kathy's group
2013-06-26 06:28:11 PM cchase \hello Elisa
2013-06-26 06:28:24 PM cchase tonight is the specialty class
2013-06-26 06:28:29 PM smmbruin10 Kathy isn't here tonight
2013-06-26 06:28:45 PM cchase here - it's the Alzheimer's chat
2013-06-26 06:28:49 PM eklemetsen Glad that I found it then!
2013-06-26 06:28:55 PM eklemetsen Hello everyone!
2013-06-26 06:28:56 PM cchase happy to have you
2013-06-26 06:29:01 PM dianajacoby Hi!
2013-06-26 06:29:03 PM eklemetsen It's good to be here!
2013-06-26 06:29:11 PM cchase how are classes going
2013-06-26 06:29:25 PM smmbruin10 also, ADC has been a really great learning opportunity. I'll be keeping my binders handy at all future jobs =)
2013-06-26 06:29:40 PM dianajacoby I'm enjoying it. It's a lot of work! Makes for long evenings and busy weekends.
2013-06-26 06:29:56 PM cchase super - glad that you are wise to use that as reference tools
2013-06-26 06:30:22 PM smmbruin10 glad that MEPAP2 is easier than MEPAP1!
2013-06-26 06:30:30 PM dianajacoby That's good to hear
2013-06-26 06:30:39 PM kdillon I am also enjoying it. My other co-worker who took a mepap class is my advisor, and she says this course seems much more helpful than the one she took.
2013-06-26 06:30:46 PM cchase so tell me about your most difficult behavior person/ resident
2013-06-26 06:32:07 PM smmbruin10 VAL! oh man, at times, this resident made it impossible to conduct group activities. she always argued with facts, called other residents names, threw things at them, etc.
2013-06-26 06:32:09 PM dianajacoby My most challenging is a gentleman who has difficulty with the group setting. He's pretty demanding for attention, and is rude to other participants. Cusses and flips them off, creating bad energy in the group.
2013-06-26 06:32:43 PM cchase what do you all do when the behavior is disruptive?
2013-06-26 06:33:17 PM kdillon we have a resident who has headed downhill fast d/t alzheimers. she gets very anxious and frustrated very easily. she started coming to us from her and her husbands cottage and is now full time in the care center. she doesn't have a long attention span, even with things she used to love doing. she wrote a few books on quilting and used to do a lot of needle work, but cant anymore. she will sit for maybe five to ten minutes with something, then get up and go
2013-06-26 06:33:28 PM smmbruin10 After a couple of years of working with her though, I knew her inside and out. i anticpated certain behaviors. learned her triggers and avoided situations that I knew would stir her up.
2013-06-26 06:33:43 PM eklemetsen I am having a hard time with getting all the reading done! Wow! I work full time as i am sure most of you do, but i am only a CNA. I have to do this on top of that and it's hard when you are a wife and mother besides
2013-06-26 06:33:49 PM dianajacoby Have the volunteer take him out of the room for a walk outside on nice days, or walk in the hallway and have 1:1 conversation.
2013-06-26 06:34:17 PM cchase good
2013-06-26 06:34:32 PM cchase I was hoping that you all took this position
2013-06-26 06:34:44 PM dianajacoby Elise, you're right. It is a lot of hard work. But good for you for sticking to it!
2013-06-26 06:34:50 PM cchase I hear of many that keep trying to keep them in the group
2013-06-26 06:35:16 PM kdillon i have found that it only frustrates a person further if you try to keep them there when they don't want to be
2013-06-26 06:35:26 PM cchase Elise - the work will pay off - I am so pleased you are giving yourself this opportunity
2013-06-26 06:35:33 PM dianajacoby We did at first. Tried ignoring him. But everyone picked up on it and he had to be removed.
2013-06-26 06:35:41 PM eklemetsen We have a lady that has Alt. and is very combative at times. So we have our work cut out for us that's for sure when trying to get her up, take her to the bathroom, and putting her to bed!
2013-06-26 06:35:49 PM cchase right- fustration but so many think that is what they should do
2013-06-26 06:36:27 PM cchase Okay got that
2013-06-26 06:36:45 PM cchase we had one like that and it was all about the approach
2013-06-26 06:37:02 PM dianajacoby I find that to be the case, too.
2013-06-26 06:37:19 PM smmbruin10 definitely
2013-06-26 06:37:31 PM cchase not that any two are remotely alike but it helps to hear a version of what worked for someone else
2013-06-26 06:37:51 PM dianajacoby I had the opportunity to attend a seminar last week by Teepa Snow on challenging behaviors. If you ever have the chance to hear her, she's amazing!
2013-06-26 06:38:17 PM cchase some folks just went up to her/him and said this is what we are doing--wrong
2013-06-26 06:38:43 PM cchase yes I have experience teepa snow - amazing
2013-06-26 06:39:18 PM cchase there was a particular way to approach that was met with a softer side of her
2013-06-26 06:39:33 PM cchase and very agreeable too
2013-06-26 06:39:36 PM dianajacoby Oh, I just thought of another challenging behavior that I'm trying to work with.
2013-06-26 06:39:44 PM cchase what
2013-06-26 06:40:29 PM dianajacoby We go on outings frequently. One gentleman is insecure when it comes to these outings.
2013-06-26 06:40:55 PM dianajacoby He needs a lot of reassurance, but can dig his heels in.
2013-06-26 06:41:18 PM dianajacoby Sometimes it works for his wife to send a note with the plans for the day.
2013-06-26 06:41:20 PM eklemetsen I found that certain people are triggers also
2013-06-26 06:41:32 PM cchase how do you introduce him to the outing?
2013-06-26 06:41:59 PM dianajacoby His wife lets him know about it first thing.
2013-06-26 06:42:07 PM dianajacoby We talk about it when everyone arrives.
2013-06-26 06:42:34 PM dianajacoby We set the stage for the day. And he's okay until it comes time to leave.
2013-06-26 06:42:45 PM cchase does he show any anxious signs while talking about it?
2013-06-26 06:42:55 PM dianajacoby Not until it's time to go.
2013-06-26 06:43:20 PM cchase is he the first to go out, last or other?
2013-06-26 06:43:31 PM dianajacoby Usually last.
2013-06-26 06:43:53 PM cchase why so - is it his choice to be last
2013-06-26 06:44:08 PM dianajacoby He'll go out with the group, but when it's time to get on the bus he says that he'll just walk over to his wife's office.
2013-06-26 06:44:49 PM cchase do you see it to be the transport vehicle
2013-06-26 06:44:53 PM dianajacoby She says that there's something about the actual bus, but we can't figure out what it is.
2013-06-26 06:45:15 PM smmbruin10 I had a similar experience. Except this resident said that if he went outside, he would be shot and killed. or that someone would rob him. most of the time, he stays behind with another activities assistant. There were a couple of times that I got him to go by just talking to him and saying that I enjoyed his company. And that he'd be safe with me. Or that it was a beautiful day and he couldn't miss it. Or I'd entice him with an outing that I knew he would enjoy.
2013-06-26 06:45:20 PM cchase have you tried to see his reaction in a regular car?
2013-06-26 06:45:28 PM eklemetsen LOL! They either think that they can walk a mile or drive where ever they need to go! We had 3 ladies that waited for the bus every day
2013-06-26 06:45:58 PM dianajacoby Haven't gone that route yet.
2013-06-26 06:46:33 PM cchase the thing with triggers is that it is a process of deduction
2013-06-26 06:46:35 PM dianajacoby That was my back-up plan for last week's outing but we were able to convince him to go with the group.
2013-06-26 06:46:51 PM dianajacoby explain that further...
2013-06-26 06:46:58 PM cchase indeed
2013-06-26 06:47:14 PM cchase it is not always the obvious
2013-06-26 06:47:31 PM eklemetsen have I been on long enough to get credit, I gotta go
2013-06-26 06:47:32 PM cchase remove one possibility at a timme
2013-06-26 06:47:35 PM smmbruin10 agreed. it can be a process
2013-06-26 06:47:48 PM cchase I will ensure your credit
2013-06-26 06:48:00 PM dianajacoby starting with?
2013-06-26 06:48:16 PM eklemetsen thanks again! I am so happy that I found you all! Nice meeting you!
2013-06-26 06:48:25 PM dianajacoby Have a nice evening, Elise!
2013-06-26 06:48:33 PM eklemetsen thanks, you too

2013-06-26 06:49:05 PM cchase start with the first sign of holding back
2013-06-26 06:49:18 PM cchase is it the group moving to the bus
2013-06-26 06:49:32 PM cchase is it the noise of all the group
2013-06-26 06:49:38 PM dianajacoby Ok. It will usually start before we go outside.
2013-06-26 06:49:39 PM cchase is it the bus itself
2013-06-26 06:49:49 PM cchase is it where he sits on the bus
2013-06-26 06:50:03 PM dianajacoby It could be the bus itself, it's not where he sits.
2013-06-26 06:50:06 PM cchase absolutely can start before he gets on the bus
2013-06-26 06:50:13 PM dianajacoby Yes.
2013-06-26 06:50:26 PM dianajacoby Fear, perhaps, of leaving the security of the room?
2013-06-26 06:50:32 PM cchase not always the obvious
2013-06-26 06:50:45 PM cchase a bus can be scary
2013-06-26 06:50:56 PM dianajacoby The unknown of where the bus may be going?
2013-06-26 06:51:00 PM cchase lots of people chatting on a bus
2013-06-26 06:51:02 PM dianajacoby That's true.
2013-06-26 06:51:22 PM dianajacoby Yes, but noise isn't a problem. I'd say it's more insecurity.
2013-06-26 06:51:30 PM smmbruin10 not sure if the bus will come back. that's what my anxios resident would say
2013-06-26 06:51:32 PM cchase do you ask them to go to the bathroom before they get on the bus
2013-06-26 06:51:35 PM dianajacoby Not being in control of his destination?
2013-06-26 06:51:37 PM kdillon left the chat room
2013-06-26 06:51:42 PM dianajacoby He does go to the bathroom before we leave.
2013-06-26 06:51:57 PM dianajacoby He waits to be the last person for the bathroom.
2013-06-26 06:52:19 PM cchase okay - he waits to go last to the bathroom - hmmm
2013-06-26 06:52:28 PM dianajacoby Therefore, he's usually at the end of the line to board.
2013-06-26 06:52:45 PM cchase does he use the bathrm often
2013-06-26 06:52:58 PM dianajacoby No, not really.
2013-06-26 06:52:59 PM smmbruin10 gotta go, goodnight. thanks for the chat

2013-06-26 06:53:25 PM cchase how about trying to get him on the bus alone
2013-06-26 06:53:26 PM dianajacoby And he always takes his lunch bag with him when we're going to go to the bus.
2013-06-26 06:53:45 PM cchase to see how he reacts
2013-06-26 06:53:53 PM dianajacoby Hmmm... could try that.
2013-06-26 06:54:01 PM cchase take him around the block and back
2013-06-26 06:54:08 PM cchase if things look good
2013-06-26 06:54:13 PM cchase not too far
2013-06-26 06:54:36 PM cchase give him praise if he does it well
2013-06-26 06:54:53 PM dianajacoby I'll give that a try. Maybe even taking him for an ice cream if he does well.
2013-06-26 06:55:05 PM cchase asked him if you could take a friend next time and build up to it
2013-06-26 06:55:24 PM cchase yes- yes - ice cream for doing well - good idea
2013-06-26 06:55:27 PM dianajacoby That's a good idea. I'll give it a try.
2013-06-26 06:55:45 PM cchase anything is possible just one thing at a time
2013-06-26 06:55:50 PM dianajacoby Just take him and a buddy and see how that works out.
2013-06-26 06:56:00 PM cchase yes - yes
2013-06-26 06:56:10 PM cchase one of his favorite friends
2013-06-26 06:56:31 PM cchase how do you advertise your support groups
2013-06-26 06:56:46 PM dianajacoby The Alzheimer's Assoc for starters.
2013-06-26 06:57:01 PM cchase where are you located?
2013-06-26 06:57:06 PM dianajacoby Eugene, OR
2013-06-26 06:57:23 PM dianajacoby The local paper's community bulletin section.
2013-06-26 06:57:24 PM cchase oh right - you said that -- OMG alz moment for me
2013-06-26 06:57:39 PM cchase I just hired a social worker
2013-06-26 06:57:43 PM dianajacoby The area agency on aging.
2013-06-26 06:57:55 PM cchase she will be very involved in the support group project
2013-06-26 06:58:02 PM dianajacoby Nice!
2013-06-26 06:58:13 PM dianajacoby In-home care agencies.
2013-06-26 06:58:27 PM cchase she is really sweet and alrady well liked by our participants
2013-06-26 06:58:39 PM dianajacoby That's a big plus!
2013-06-26 06:58:45 PM dianajacoby Where are you located?
2013-06-26 06:58:53 PM cchase okay got it- do you specify the topic or do they decide
2013-06-26 06:58:57 PM cchase Mass
2013-06-26 06:59:10 PM dianajacoby The topic is determined by the need for the week.
2013-06-26 06:59:20 PM cchase what is the need for the week
2013-06-26 06:59:27 PM dianajacoby I used to pre plan a topic, but it wasn't always relevant.
2013-06-26 06:59:58 PM cchase how do you determine the need for the week
2013-06-26 06:59:59 PM dianajacoby That's determined by the group from week-to-week. What's the greatest issue brought in that week.
2013-06-26 07:00:29 PM cchase group from week-to-week??
2013-06-26 07:00:40 PM dianajacoby Sometimes it will be covered the following week if I need to research something.
2013-06-26 07:01:02 PM dianajacoby Pretty much.
2013-06-26 07:01:41 PM cchase we offer refreshments and make it an opportunity for caregivers to get to know each other
2013-06-26 07:02:16 PM cchase they reach out to each other and are elated to know that someone else is having the same things to deal with
2013-06-26 07:02:20 PM dianajacoby That's great! Most of our groups meet right after lunch.
2013-06-26 07:02:28 PM dianajacoby Very true!
2013-06-26 07:02:55 PM dianajacoby How long do they last? 1 hr., 2 hrs.?
2013-06-26 07:03:28 PM cchase I prefer no longer than 1 1/2 hrs... the last half is an open social
2013-06-26 07:03:41 PM cchase half hour is an open social
2013-06-26 07:03:42 PM dianajacoby How many attend?
2013-06-26 07:04:03 PM dianajacoby I like the open social for the last 1/2 hour idea.
2013-06-26 07:04:24 PM dianajacoby How do you cover cost of facilitation
2013-06-26 07:04:54 PM cchase yes - they seem too as well...we started with 3 and now have an average of 12 - 15
2013-06-26 07:05:13 PM dianajacoby That's about what ours are, too.
2013-06-26 07:05:33 PM cchase we also have special presentors and have anywhere btw 15 - 20's
2013-06-26 07:06:11 PM dianajacoby We have an educational series that is separate from the support groups. This is where we have the special presenters.
2013-06-26 07:06:17 PM cchase we cover the cost of refreshments but have been offered by those that attend to help with that
2013-06-26 07:06:36 PM cchase yes - sounds very similar to what we do
2013-06-26 07:06:57 PM dianajacoby Where do the funds come from to cover cost of facilitation?
2013-06-26 07:07:28 PM cchase we send out mailers/emails - to invite also constant contacts on web
2013-06-26 07:07:29 PM dianajacoby We used to get funding from OAA, but our contract was cancelled for 2013-2014 because of the sequestration.
2013-06-26 07:07:48 PM cchase interesting
2013-06-26 07:08:11 PM cchase we have been fortunate to work out more of an exchange situation
2013-06-26 07:08:24 PM dianajacoby How does that work?
2013-06-26 07:09:05 PM cchase we help other professionals network and offer there wares (sort to speak) for free in exchange for there time to present
2013-06-26 07:09:31 PM cchase we now have a sort of waiting list
2013-06-26 07:10:02 PM cchase if presentors
2013-06-26 07:10:22 PM dianajacoby Hmmm... That's definitely something for us to consider. We don't want to cut back or eliminate any of the groups, but need to have $$ for facilitation.
2013-06-26 07:11:23 PM dianajacoby It's getting late for you on the east coast, so I'll let you get going. I"ll try to make your next session on July 3rd! Thanks for all the tips and have a nice weekend!
