2020-07-08 06:18:01 PM jessiev1 Hey!
2020-07-08 06:18:22 PM smithtammyl it's a one hour class
2020-07-08 06:18:22 PM diovanneb @jjmiller227 Yes, I do.
2020-07-08 06:18:24 PM lyndsayt I find it very hard to do activities with people with MS because they have such limited movements.
2020-07-08 06:18:34 PM jjmiller227 diovanneb - please let us know your name so we can add to the list
2020-07-08 06:18:37 PM diovanneb that's right
2020-07-08 06:18:55 PM smithtammyl left the chat room
2020-07-08 06:18:57 PM diovanneb I'm Diovanne Barlow, from VA
2020-07-08 06:19:04 PM jessiev1 MS, I agree.
2020-07-08 06:19:10 PM lwilliams10 I find it just depends on the resident and how involved they are willing to be with groups. I have some residents tell me right off the bat when i do their assesment that they do not want to do any gorups what so ever and tehn others that are eager to get involved.
2020-07-08 06:19:39 PM jessiev1 'Historical Loner' is also difficult for me.
2020-07-08 06:19:53 PM diovanneb Cprrect, there are residents especially those with MS that they rather do 1:1 than attend grps
2020-07-08 06:19:59 PM paula12 I have the same issue they tell meone thing and tell others differently
2020-07-08 06:20:07 PM lwilliams10 We have a secured demntia unti at my facility and their activities are so much different from the other halls of my building.
2020-07-08 06:20:10 PM jessiev1 Yes!
2020-07-08 06:20:36 PM lyndsayt My name is LyndsayTalbott by the way. I need to be on the list.
2020-07-08 06:20:46 PM paula12 Secured dementia can be very difficult
2020-07-08 06:21:01 PM lyndsayt Whatt does CAA stand for?
2020-07-08 06:21:04 PM diovanneb In my facility, I find it a little challenging when do have mixed groups, there are alert and oriented and some with dementia or alzheimer
2020-07-08 06:21:10 PM michaeld enter the chat room
2020-07-08 06:21:23 PM jessiev1 Yes it can. I used to work on a secured Traumatic Brain Injury unit, that was tough too.
2020-07-08 06:21:30 PM lwilliams10 Yes it is much harder to keep them focused on the groups.
2020-07-08 06:21:30 PM paula12 I agree mixed groups can be tough
2020-07-08 06:21:38 PM jjmiller227 what do you care plan for the historical loner? Thanks Lyndsay T
2020-07-08 06:21:39 PM michaeld Hello all. Sorry im late
2020-07-08 06:21:51 PM diovanneb I think CAA means CARE AREA ASSESSMENT
2020-07-08 06:22:04 PM lyndsayt Oh thank you!
2020-07-08 06:22:14 PM paula12 The historical loner i usually care plan one on ones
2020-07-08 06:22:46 PM jjmiller227 Micaeld - give us your full name to add to the list. Thanks
2020-07-08 06:22:47 PM diovanneb Yes, we can do 1:1 with historical loners
2020-07-08 06:22:58 PM michaeld Michael Deloach
2020-07-08 06:23:01 PM lwilliams10 I careplan what independant activities they like to do daily on their own and then careplan that one on one room visits will be provided at least 2 x a week
2020-07-08 06:23:17 PM jjmiller227 Thanks Michael
2020-07-08 06:23:27 PM paula12 I do the same
2020-07-08 06:23:55 PM jessiev1 1:1, Encourage and praise attendance, offer invitations to programs outside of assessed preferences
2020-07-08 06:23:57 PM diovanneb I know when we do care plan we have to be specific and personalized but what if we cannot visit the resident 2 or 3x a week...?
2020-07-08 06:24:07 PM lyndsayt What do you do when residents are gruff and want nothing to do with you or activities? Any tips to get them to open up?
2020-07-08 06:24:08 PM diovanneb what do we write in the care plan then?
2020-07-08 06:24:18 PM lwilliams10 i also put that will will still invite/encourage groups
2020-07-08 06:24:43 PM paula12 I visit the resident as often as I can and hope they will come around
2020-07-08 06:24:50 PM diovanneb @lyndsayt - social conversation first...
2020-07-08 06:24:56 PM jjmiller227 Are there any independent activities that you can offer to them?
2020-07-08 06:25:03 PM jessiev1 'gruff', we still socialize at least three times a week.
2020-07-08 06:25:06 PM diovanneb allow them to talk about themselves
2020-07-08 06:25:07 PM lwilliams10 At my facility if a resident does not attend groups wee have to provide one on ones or we have been sited by state in previous years before i took over.
2020-07-08 06:25:21 PM stargie69 enter the chat room
2020-07-08 06:25:39 PM jessiev1 Is anybody here allowed to do group activities right now?
2020-07-08 06:25:42 PM lwilliams10 just chatting with them 5 to 10 minutes can count as a one on one
2020-07-08 06:25:54 PM paula12 When a resident comes in and does not want to be included in activities we give them a bag of things for them to do
2020-07-08 06:26:02 PM diovanneb @jessiev1 - in my facility no grp activities yet
2020-07-08 06:26:16 PM lwilliams10 the closest thing to a group that i can do is hallway bingo. they have to stay in their doorway and have mask on.
2020-07-08 06:26:17 PM diovanneb until we get the results from our COVID test
2020-07-08 06:26:20 PM lyndsayt @paula2 thats a great idea
2020-07-08 06:26:43 PM paula12 No group activites unless we are outside and everyone needs to be at their own table
2020-07-08 06:26:52 PM jjmiller227 I also like that idea Paula
2020-07-08 06:27:02 PM diovanneb @paula12 - yes, still 6 ft apart
2020-07-08 06:27:23 PM lyndsayt We have groups of 10 or less, wearing masks, 6ft apart. Inside or outside
2020-07-08 06:27:27 PM lwilliams10 we have been doing activity packets for the residents and including word search puzzles, coloring pages, trivia, different articiles to read
2020-07-08 06:27:33 PM paula12 We just did bingo today outside we have a microphone which helps
2020-07-08 06:27:47 PM diovanneb @lwilliams10 - yes we do that too!
2020-07-08 06:28:00 PM jjmiller227 outside bingo, that would be fun
2020-07-08 06:28:16 PM michaeld We have three units. No one unit can mix.
2020-07-08 06:28:24 PM diovanneb hallway bingo and hallway sing-a-long are great ideas
2020-07-08 06:28:54 PM paula12 we play JINGO and our residents love it
2020-07-08 06:29:03 PM jessiev1 No groups at my facilityy either, but we have 'regulars' that visit everyday and we only let 1 person per table.
2020-07-08 06:29:04 PM lyndsayt We have been doing hallway parades with carnival games, treats, music, staff in costumes, etc...
2020-07-08 06:29:24 PM jjmiller227 What are some other challenges you have with the MDS?
2020-07-08 06:29:25 PM lwilliams10 We have also done hallway balloon volley. We got pool noodles and give to each resident and they hit balloons up and down the halls to eachother while staying in their doorways.
2020-07-08 06:29:56 PM diovanneb hallway balloon volley sounds fun!
2020-07-08 06:30:01 PM paula12 Sing a longs in the hallway are always fun
2020-07-08 06:30:32 PM michaeld My have issue with when a patient is a readmit
2020-07-08 06:30:58 PM jjmiller227 In what ways Michael?
2020-07-08 06:31:02 PM paula12 We just started happy hour Fridays we make slushies and get a fun snack and decorate our cart and go around to all of the residents
2020-07-08 06:31:05 PM diovanneb @michaeld - what issues do you encounter with readmits?
2020-07-08 06:31:10 PM lwilliams10 @micheld what issues do you have?

2020-07-08 06:32:17 PM jjmiller227 Hi stargie69! What type of facility are you at and where?
2020-07-08 06:32:18 PM michaeld "Significant changes"
2020-07-08 06:32:30 PM stargie69 Hello
2020-07-08 06:32:32 PM paula12 When we get a readmitt they have to be quarantined for 14 days.
2020-07-08 06:33:01 PM paula12 so they get a bag of activities to keep them busy
2020-07-08 06:33:02 PM jjmiller227 How are you able to do your MDS with quarantined residents?
2020-07-08 06:33:03 PM diovanneb @michaeld - yes, we have to do assessment all over again
2020-07-08 06:33:26 PM stargie69 I worked at facility with 120 bed but due to covid-19 sencus down to 74
2020-07-08 06:33:38 PM diovanneb diagnosis are different, and ability to answer questions too
2020-07-08 06:33:40 PM paula12 We suit up in our PPE and go and do our MDS
2020-07-08 06:33:45 PM stargie69 Im here in Florida
2020-07-08 06:34:25 PM lwilliams10 If they are a significant change we review there last current assessment to see if anything has changed as far as Activities
2020-07-08 06:34:41 PM diovanneb sometimes I call family members if the resident can not provide info because of the change
2020-07-08 06:34:42 PM lyndsayt What does MDS stand for?
2020-07-08 06:34:42 PM paula12 What position do you all have at your facility
2020-07-08 06:34:53 PM michaeld Ok i see
2020-07-08 06:35:09 PM lwilliams10 Family is a big help.
2020-07-08 06:35:20 PM diovanneb MINIMUM DATA SET
2020-07-08 06:35:33 PM jessiev1 Paula12, not all 'R' are able to do independent recreation, so a bag of activities isn't always efecient.
2020-07-08 06:35:41 PM lwilliams10 I am the Activity Director
2020-07-08 06:36:18 PM michaeld The ones that have no family can be a little difficult at times to assest
2020-07-08 06:36:36 PM diovanneb @michaeld - but if there's no family member or you can't reach them, CNAs and Nurses can answer questions too if they know the resident.
2020-07-08 06:36:45 PM lwilliams10 yes that is true.
2020-07-08 06:36:46 PM paula12 We go to those residents and do activites with them even if they are quarantined
2020-07-08 06:37:02 PM jjmiller227 Yes CNA/STNAs are invaluable sources
2020-07-08 06:37:05 PM michaeld diovanneb correct
2020-07-08 06:37:16 PM diovanneb there's a part in the questionnaire where we can interview "staff"
2020-07-08 06:37:26 PM lwilliams10 We do also unless they would have COVID but luckily we havnt had any cases at my building yet!!
2020-07-08 06:37:43 PM michaeld diovanneb right
2020-07-08 06:37:52 PM jessiev1 We haven't either so far!
2020-07-08 06:38:14 PM paula12 we have had covid at my building
2020-07-08 06:38:17 PM jjmiller227 You are very fortunate
2020-07-08 06:38:20 PM diovanneb but "staff" will be the last resort, only if there's no family members...we still have to write it in our documentation that we tried to reach for the family members.
2020-07-08 06:39:08 PM diovanneb @lwilliams10 - you are lucky. We had in our building.
2020-07-08 06:39:42 PM jjmiller227 What are some other challenges you have with the MDS? With all that you do, I don't know how you have the time....
2020-07-08 06:39:43 PM lwilliams10 @diovanneb Sorry to hear that- hopefully it is getting better
2020-07-08 06:40:23 PM paula12 We have not had an active case at our building since April 20
2020-07-08 06:40:38 PM jjmiller227 That is great news!
2020-07-08 06:40:52 PM diovanneb We had about 14 residents but all from Assisted L.
2020-07-08 06:41:03 PM jessiev1 Care conferences take more of my time than MDS documentation. I regularly have 8 a day on tuesdays and thursdays.
2020-07-08 06:41:16 PM diovanneb But now the issue is not COVID anymore but depression
2020-07-08 06:41:18 PM michaeld diovanneb Lucky for me i have two assistance that carry out the activities while i do all the charting
2020-07-08 06:41:58 PM lwilliams10 Care Conferences put a damper in my day. We alsways have them mid day, 4 days a week 4-5 residents a day
2020-07-08 06:41:58 PM jjmiller227 Diovanne - can you explain more?
2020-07-08 06:41:59 PM stargie69 wow you are lucky then
2020-07-08 06:42:07 PM paula12 i do both and it does take alot of time
2020-07-08 06:42:13 PM diovanneb @michaeld - heyyyy! I want that too! In my facility I do have 2 others who work with me, but I still do activities as well!
2020-07-08 06:42:24 PM lyndsayt Depression is a major issue at our facility right now
2020-07-08 06:42:55 PM jjmiller227 Yes I would agree. We've seen cognitive decline as well
2020-07-08 06:42:56 PM diovanneb @jjmiller227 - currently we don't have any positive COVID anymore...for the last 1 month
2020-07-08 06:43:11 PM diovanneb but residents are getting depressed more of not seeing family members
2020-07-08 06:43:22 PM diovanneb in my facility we are still on phase1
2020-07-08 06:43:24 PM stargie69 i agree
2020-07-08 06:43:24 PM jessiev1 Yes! That's why Social Services always triggers Activities in my facility, depression.
2020-07-08 06:43:29 PM jjmiller227 yes and not being able to come/go as they had
2020-07-08 06:43:33 PM paula12 our residents are depressed as well
2020-07-08 06:43:38 PM lwilliams10 We are getting depression also and the main reason i believe is becuase our residents cannot physically touch their families when the see them.
2020-07-08 06:43:51 PM paula12 i agree
2020-07-08 06:44:00 PM diovanneb i agree
2020-07-08 06:44:00 PM stargie69 you are right about that
2020-07-08 06:44:05 PM jessiev1 yep!
2020-07-08 06:44:50 PM diovanneb also, another thing is when we see our residents depressed when they were used to be so active and joyful...we also get affected
2020-07-08 06:45:07 PM stargie69 true..
2020-07-08 06:45:11 PM nancylee123 enter the chat room
2020-07-08 06:45:11 PM diovanneb burnout
2020-07-08 06:45:32 PM jjmiller227 It is so sad to think we're going to be at this for long time and no matter how much we do to engage and keep them active and spirits up, nothing can fill the gap with family contact
2020-07-08 06:45:49 PM lwilliams10 Yes it is sad. I cant wait for the day we can get back to our new "normal"
2020-07-08 06:45:54 PM diovanneb that's true
2020-07-08 06:45:59 PM stargie69 absolutely right
2020-07-08 06:46:34 PM jessiev1 We're all in uncharted territory and doing the best we can.
2020-07-08 06:46:38 PM diovanneb I have a resident whose spouse passed away beginning of covid restrictions...then he couldn't see his family members because they were not allowed in
2020-07-08 06:46:45 PM jjmiller227 Any approaches we can do to fill in that gap?
2020-07-08 06:47:04 PM diovanneb i felt bad for him and i still do...
2020-07-08 06:47:04 PM jjmiller227 Oh that is so heartbreaking...
2020-07-08 06:47:40 PM diovanneb I helps him with facetime with his family members but that is not the same as seeing them and touching them
2020-07-08 06:47:41 PM paula12 its very hard when a spouse dies .I had a residents mom oass away it was heartbreaking how sad he is
2020-07-08 06:47:49 PM lwilliams10 We have families that do window visits and then an IPAD for virtural visits but some residents dont understand why there families are on a screen
2020-07-08 06:48:08 PM paula12 That is a real problem
2020-07-08 06:48:27 PM lyndsayt My residets think they are being punished. Breaks my heart
2020-07-08 06:48:44 PM stargie69 that is sad...
2020-07-08 06:48:55 PM jessiev1 Most of mine understand, but it is still affecting them.
2020-07-08 06:48:56 PM diovanneb yes it is heartbreaking
2020-07-08 06:49:02 PM jjmiller227 How are you keeping your spirits up during all this?
2020-07-08 06:49:05 PM paula12 I had one resident kiss the ipad and he said why isnt he kissing me back
2020-07-08 06:49:35 PM diovanneb after all this we too have to take care of ourselves
2020-07-08 06:49:50 PM jjmiller227 Yes we do!
2020-07-08 06:49:55 PM paula12 i agree
2020-07-08 06:49:58 PM michaeld We had "Cabana Visit". We had a hug acrylic box built on our front patio. Where we push the resident in and there family can see the resident in there fullnest. But DHEC made us close it down about two weeks ago because the cases were getting higher. So our resident have become more depress. Our windows are to high off the ground for window visits. So mostly phone and facetime now.
2020-07-08 06:50:11 PM diovanneb sometimes I just cry...we have to let it out and cant keep it in
2020-07-08 06:50:46 PM lwilliams10 i have sat with many residents while they cry because we are the closest thing they have to family right now.
2020-07-08 06:50:55 PM jjmiller227 cabana visit was a great idea
2020-07-08 06:51:19 PM jjmiller227 yes we are their family
2020-07-08 06:51:25 PM diovanneb now we are on phase 1, we have the cabana visit too
2020-07-08 06:51:27 PM paula12 we are now doing visits over a fence in our courtyard
2020-07-08 06:52:15 PM diovanneb we tried to do that too paula12...
2020-07-08 06:52:31 PM diovanneb but family members can't avoid themselves from hugging the residents
2020-07-08 06:52:41 PM lwilliams10 For Ohi July 22nd we can start outdoor visits whiloe still following guidelines per the govenor but it still is up to each faciltiy if they want to do them or not.My facility has not come up with their plan yet.
2020-07-08 06:52:42 PM diovanneb so they stopped it
2020-07-08 06:53:51 PM jjmiller227 Our MCU residents don't want to keep the masks on during outside visits, even some family members are a challenge
2020-07-08 06:54:05 PM lynnh enter the chat room
2020-07-08 06:54:15 PM diovanneb yes
2020-07-08 06:54:17 PM stargie69 yes that is very true
2020-07-08 06:54:40 PM jessiev1 One of my staff members has a TBI son in another facility, he talks to me about how hard it is for him as parent, only being able to talk to his son through a window. Gives me a perspective of both sides.
2020-07-08 06:54:44 PM paula12 our family members have to sign a list of rules and if they dont follow them they can not visit anymore
2020-07-08 06:54:55 PM michaeld Yes family can be difficult at times
2020-07-08 06:55:42 PM jjmiller227 Stargie - what is your full name so I can get it on the list?
2020-07-08 06:55:43 PM michaeld paula12 Same at my facilty
2020-07-08 06:55:51 PM lwilliams10 we have had some family members cut the screens on the windows and open them and climb through the windows to try and visit. Its hard but they dont understnad they are exposing everyone not just their familoy member or themselves
2020-07-08 06:55:53 PM stargie69 Grace White
2020-07-08 06:56:09 PM michaeld lwilliams10 OMG!!!
2020-07-08 06:56:19 PM paula12 thats horrible
2020-07-08 06:56:31 PM lynnh we have had that too
2020-07-08 06:56:36 PM diovanneb Somehow we never ever thought of this situation...how did we get here. I feel bad for the family members too...
2020-07-08 06:56:51 PM jjmiller227 Oh my goodness. Back to the MDS - what tips would you give to a new AD?
2020-07-08 06:57:00 PM michaeld diovanneb Me too
2020-07-08 06:57:19 PM jessiev1 Tips? Know your residents.
2020-07-08 06:57:39 PM diovanneb yes, know your residents.
2020-07-08 06:57:49 PM michaeld jjmiller227 Have a open mind. Thing are always changing.
2020-07-08 06:58:01 PM lwilliams10 take advantage when doing the MDS assessment. be indepth with the questions to get more info so you have a better idea of things and groups to offer them.
2020-07-08 06:58:22 PM lynnh if they have glasses make sure they are on and hearing aods too
2020-07-08 06:58:32 PM diovanneb spend time with them too...I know most AD do not do activities, but if you spend some time with them you'll get to know them
2020-07-08 06:58:33 PM lwilliams10 KNow that not every resident will be the same nor will on e resident want to do the same things all the time.
2020-07-08 06:58:44 PM paula12 visit your residents as often as you can and get to know them
2020-07-08 06:58:57 PM jessiev1 I make a point every week to do nothing but spend at least an hour away from all the charting and with my residents. If I don't know them, how can I properly serve them?

2020-07-09 04:53:34 PM travesmith its important to build trust between you and your residents. You can do so by practicing healthy conversations with meaningful dialog and going the extra mile to help them in away that you can even if its not within your job discription.
2020-07-09 04:55:32 PM travesmith Age appropriate activities are important. Especially in a short term, skilled nursing home or rehab center. They tend to have a variety of people aging from young to older.
