2020-07-11 02:04:45 PM paulapoole hello
2020-07-11 02:04:58 PM susandziegenbein Yes
2020-07-11 02:05:01 PM paulapoole I hope so, that's what I'm here for
2020-07-11 02:05:05 PM turnerjennifer knowing the diagnosis of a resident definitely helps drive your careplan
2020-07-11 02:05:23 PM barbaramclaurin1 The information in the active diagnosis list will also let workers know what is currently going on with the individual.
2020-07-11 02:05:23 PM suzannemmartin It is good to know what the diagnoses are so we can see how much a resident can function for activities
2020-07-11 02:05:35 PM lbaker1989 I agree! Knowing your residents information will help make a better careplan
2020-07-11 02:05:40 PM spassa yes it does.... and we have to make sure peoples needs fit their careplan
2020-07-11 02:05:51 PM lynnh agree
2020-07-11 02:05:53 PM turnerjennifer And some of those diagnosis will also explain behaviors they may be having
2020-07-11 02:05:55 PM susandziegenbein It will help to know their limitatins and strengths
2020-07-11 02:05:56 PM atirado Great for resident centered care!
2020-07-11 02:05:58 PM maryneals48 agree

2020-07-11 02:06:28 PM suzannemmartin I am at a SNF and Rehab center, we have people from 18 to 100 with all types of issues
2020-07-11 02:06:32 PM spassa you also know we gotta check section v to see if activities was triggered
2020-07-11 02:06:36 PM susandziegenbein Greater understanding of the mental/physical/behavioral needs and issues
2020-07-11 02:06:51 PM lbaker1989 I like to know the residents behaviors and their diagnoses to help me better understand the resident
2020-07-11 02:06:57 PM barbaramclaurin1 It will ensure that residents are being monitored appropriately for current symptoms and not older or resolved diagnoses.
2020-07-11 02:07:07 PM turnerjennifer a lot of directors I have I helped out always forget to look at section V
2020-07-11 02:07:08 PM lbaker1989 then we can make programs better for their needs!
2020-07-11 02:07:23 PM atirado This is true for my unit which is the Alzheimer's/Dementia.
2020-07-11 02:07:42 PM suzannemmartin I have to do sections F and V, but only if V is triggered
2020-07-11 02:07:46 PM terrycierra I've worked in both settings and I can say this list definitely helps with person-centered care!
2020-07-11 02:07:46 PM maryneals48 yes knowing your resident helps alot
2020-07-11 02:07:50 PM lbaker1989 Does anyone else just fill out the F section? All I have to fill out is the F section, but some places you have to do other sections
2020-07-11 02:07:54 PM victoriasmith81 enter the chat room
2020-07-11 02:07:59 PM spassa same here susanne
2020-07-11 02:08:00 PM barbaramclaurin1 I agree then activities can be developed or planned based on current functional abilty and status of the indvidual.
2020-07-11 02:08:03 PM turnerjennifer I only have to do section F
2020-07-11 02:08:08 PM turnerjennifer and V if it triggers
2020-07-11 02:08:14 PM lbaker1989 ok!

2020-07-11 02:08:37 PM atirado When I get a new admission I do several assesments. With the resdent, family, nursing and social services.
2020-07-11 02:08:42 PM turnerjennifer I've always loved doing MDS
2020-07-11 02:08:45 PM spassa and thats usually through a CAA worksheet
2020-07-11 02:09:20 PM paulapoole I'm in a LTCF, SNF, Rehab. I do sections F and V.
2020-07-11 02:09:40 PM atirado I like to still interview the resident no matter cognative level to still make sure they are apart of there care plan.

2020-07-11 02:09:54 PM clcroom enter the chat room
2020-07-11 02:09:55 PM lbaker1989 I have my team do the interviews
2020-07-11 02:10:00 PM terrycierra Currently, I am an activity director in an ALF--we do not use MDS sheets
2020-07-11 02:10:05 PM barbaramclaurin1 Cognitive ability and status is important to know for each resident so that activities can be based on a person's ability to function cognitively.
2020-07-11 02:10:08 PM lbaker1989 because they work with them on a daily basis
2020-07-11 02:10:09 PM turnerjennifer write down all of the names and one person is to post it

2020-07-11 02:13:05 PM barbaramclaurin1 I am a senior center director and we do not do MDS sheets. We do record diagnoses based on the individual report. I believe that having an active MDS sheet would better assist us with managing behaviors. So I will definately be updating our approach to recording information.

2020-07-11 02:15:52 PM terrycierra LOVE visual scales
2020-07-11 02:16:10 PM atirado Depending on the person I use flash cards to minimize info being processed and larger print for visual imparment.

2020-07-11 02:16:41 PM lbaker1989 like flash cards or pictures to help the resident understand better

2020-07-11 02:17:04 PM lbaker1989 :)

2020-07-11 02:17:22 PM victoriasmith81 did you make the cards for it
2020-07-11 02:17:26 PM paulapoole Sometimes I have to give examples and remind them as to their stay here vs. being at home

2020-07-11 02:17:38 PM susandziegenbein Visual scales would seem to help with those having difficulty communication. Such a stroke victims
2020-07-11 02:17:53 PM terrycierra my last company used MATRIX
2020-07-11 02:18:11 PM lauralehmann yes PCC
2020-07-11 02:18:14 PM atirado I've always used pcc...
2020-07-11 02:18:21 PM paulapoole always pcc
2020-07-11 02:18:23 PM turnerjennifer Does anyone do a meeting with interdisplinary team to go over MDS
2020-07-11 02:18:23 PM spassa pcc
2020-07-11 02:18:27 PM maryneals48 always pcc
2020-07-11 02:18:31 PM paulapoole yes
2020-07-11 02:19:07 PM atirado but PCC can be diffrent. The set up was diffrent in my last job as an aid than it is at my current.
2020-07-11 02:19:08 PM lbaker1989 Thats good to know! Yes, we have daily clinicals, but I dont attend those meetings
2020-07-11 02:19:09 PM suzannemmartin I have only been included in a few meetings for activity info
2020-07-11 02:19:13 PM stargie69 we never put 1.1 we use the term friendly room visit
2020-07-11 02:19:19 PM turnerjennifer Is it required or just something your building does?
2020-07-11 02:19:23 PM spassa we do Quapi meeting . This is where the Doctor of facility gets to hear all the areas of faciltiy to say its safe and needs are met
2020-07-11 02:19:31 PM suzannemmartin we use room visit as well
2020-07-11 02:19:41 PM lbaker1989 I like that better than one on one
2020-07-11 02:19:55 PM lauralehmann yes,,,always meeting with mds and the managment team always meets with all new admits to get a care plan established
2020-07-11 02:19:57 PM turnerjennifer The last SNF I was at we did a behavior meeting but we would discuss MDSs in there too
2020-07-11 02:20:39 PM lauralehmann we do quapi also 1 time a month with the house doctor...usually a luncheon
2020-07-11 02:20:40 PM suzannemmartin I am getting better at navigating MDS but still learning each day
2020-07-11 02:20:41 PM atirado We do 1:1's for low activity participation if nothing 2-3 times pr week and we also do friendly visits for residents who are more independent or cannot do a whole min 15 min 1:1.
2020-07-11 02:20:43 PM lynnh as assistants we dont know what they dicuss in the meetings or from our Activity Director
2020-07-11 02:20:48 PM turnerjennifer Off of the list of active diagnosis, what is the most common you see at your facility
2020-07-11 02:20:53 PM paulapoole I like the term room visits better too, since we don't have "room visits" as a choice in PCC we have to put 1:1, especially since Covid. Put room visits don't define 1:1s
2020-07-11 02:21:06 PM lynnh UTI
2020-07-11 02:21:28 PM suzannemmartin As an AD I share with my staff things we talk about so they can understand what is going on with the residents
2020-07-11 02:21:35 PM terrycierra def uti
2020-07-11 02:21:39 PM lbaker1989 I do that too
2020-07-11 02:21:46 PM paulapoole Me too.
2020-07-11 02:21:48 PM lbaker1989 yes Uti
2020-07-11 02:21:50 PM maryneals48 I do that too
2020-07-11 02:21:55 PM turnerjennifer Afib, CAD, CHF, and UTI are common ones I've seen
2020-07-11 02:22:04 PM lauralehmann as an assisitant, your activity directer should always discuss things with you
2020-07-11 02:22:05 PM atirado Our issues is we only have one button in pcc for 1:1/friendly visits so we also do paper documintaion as a back up for state.
2020-07-11 02:22:27 PM lynnh we as assistants only
2020-07-11 02:22:28 PM spassa my area is copd . stroke afib and i don't usually see the uti
2020-07-11 02:22:36 PM lynnh Dementia
2020-07-11 02:22:40 PM stargie69 yes my director always inform me whats going on at all time
2020-07-11 02:22:49 PM turnerjennifer agree
2020-07-11 02:22:50 PM suzannemmartin the uti causes all kinds of side effects
2020-07-11 02:22:56 PM terrycierra good! communication is #1
2020-07-11 02:22:59 PM stargie69 yes
2020-07-11 02:23:02 PM atirado Yes!
2020-07-11 02:23:02 PM turnerjennifer UTI is a nightmare in the elderly!!!!!
2020-07-11 02:23:09 PM terrycierra true
2020-07-11 02:23:18 PM susandziegenbein yes
2020-07-11 02:23:20 PM lbaker1989 We do have a lot of dementia, COPD and Uti
2020-07-11 02:23:21 PM paulapoole Parkinsons, demintia
2020-07-11 02:23:30 PM maryneals48 yes
2020-07-11 02:23:31 PM turnerjennifer how long does this chat need to last?
2020-07-11 02:23:37 PM atirado 1hr
2020-07-11 02:23:40 PM barbaramclaurin1 I think that knowing a person's active diagnosis is paramount. In my facility, we do write ups based on a person's behavior and I have noticed participants treated in such a way by staff with no respect to a person's diagnosis.
2020-07-11 02:23:53 PM lbaker1989 i agree
2020-07-11 02:24:00 PM atirado Patient canter care!
2020-07-11 02:24:06 PM spassa I think its an hour for credit but not sure what all needs to cover for it didnt give a list
2020-07-11 02:24:24 PM maryneals48 1hr
2020-07-11 02:24:28 PM suzannemmartin just going over MDS 3.0 active diagnoses
2020-07-11 02:25:03 PM atirado I think a half hour is enough but it's 1 hr...
2020-07-11 02:25:07 PM paulapoole We try our best to be as resident centered as possible. We have to alot a CNA repair for pychosocial wellbeing though
2020-07-11 02:25:12 PM turnerjennifer Looking at some of the active diagnosis and some of them doesn't seem like it would affect activities much
2020-07-11 02:25:20 PM spassa so thinking of those lower functioning diagnoses how do you help reach them... i use alot of mp3 and sensory
2020-07-11 02:25:21 PM suzannemmartin I think having some background on the resident helps us in activities to offer different choices based on things they liked to do in their past
2020-07-11 02:25:42 PM terrycierra interestingly, i find that working in my alf that communication (specifically on diagnosis and health history) is lacking. unless i specifically ask the head nurse on my units, no info is given.
2020-07-11 02:25:48 PM atirado I love sensory. I'm a huge fan of aromatherapy!
2020-07-11 02:25:56 PM lbaker1989 me too!
2020-07-11 02:25:56 PM turnerjennifer We got a jukebox donated one time and I used that a lot
2020-07-11 02:26:01 PM lbaker1989 nice!
2020-07-11 02:26:09 PM lbaker1989 i wish we had one!
2020-07-11 02:26:10 PM terrycierra aromatherapy is huge in memory care!
2020-07-11 02:26:11 PM lynnh It helps me since I was a CNA then transferred to Activities so i have a good communication with the nurses
2020-07-11 02:26:11 PM stargie69 wow..
2020-07-11 02:26:13 PM suzannemmartin Residents that became quad's still like to talk about or draw things that they enjoyed doing, like hiking or sight seeing
2020-07-11 02:26:22 PM barbaramclaurin1 In my facility we see some dementia, mental illness, many have medical issues. But for the most part my participants are higher functioning. Many drive to the facility.
2020-07-11 02:26:44 PM turnerjennifer barbara what type of facility?
2020-07-11 02:26:49 PM barbaramclaurin1 I still believe that knowing active diagnoses will assist with planning.
2020-07-11 02:26:53 PM lynnh we are a mid to a low facility
2020-07-11 02:27:10 PM suzannemmartin we had some dementia residents try and eat the items we were trying to get them to smell
2020-07-11 02:27:22 PM barbaramclaurin1 Senior Center like a day program.@jenniferturner
2020-07-11 02:27:24 PM lbaker1989 same here!
2020-07-11 02:27:29 PM atirado Yes! I use a difuser!
2020-07-11 02:27:30 PM stargie69 mine is rehab and long term
2020-07-11 02:27:37 PM lynnh they sure casn be fast and sneakey
2020-07-11 02:27:38 PM lbaker1989 We had to hold the items for them so they wouldnt eat it
2020-07-11 02:27:39 PM victoriasmith81 our activity director walked out one day no notice and i was thrown into i love it but needs so many ideas and help we are a skilled nursing with 60 residents and i do it alone.
2020-07-11 02:27:47 PM maryneals48 rehab and long term care
2020-07-11 02:27:49 PM clcroom Perhaps strongly scented food? They could do both.
2020-07-11 02:27:50 PM spassa I have used mp3 players and put on different music for different patients needs. also can use the Ipads and do games that residents can play against each other
2020-07-11 02:27:52 PM atirado But we use alot of food items and stuff that is non toxic all natral
2020-07-11 02:27:58 PM paulapoole I think you should some idea about the diagnosis too. For example, people need to understand the those with parkinsons can still understand everything you're saying... they just can't get their body to respond like it should. Therefore, staff and families who stand around these precious ppl like they're twits frustrate me so much and I feel I need to educated them (graciously of course)
2020-07-11 02:28:19 PM susandziegenbein Not being in the alf or snf. This will be a major job transition for me. What do you think I would see the most in residents? What will be the most suprising thing be to me being new in the fileld?
2020-07-11 02:28:22 PM lynnh so are the MDS part of a progress note so new to this
2020-07-11 02:28:24 PM terrycierra difussers and rollers are great!
2020-07-11 02:28:28 PM suzannemmartin yes sometimes they lose their sense of smell and they don't want to eat anymore, so the more smelly we can make the items the better
2020-07-11 02:28:29 PM barbaramclaurin1 @paulapoole, I agree.
2020-07-11 02:28:34 PM spassa I like putting things in boxes and let them place hand in and they can guess what it is
2020-07-11 02:28:37 PM turnerjennifer agree Paula
