2020-07-15 07:09:04 PM davislsc is used to help create an activity based on the residents diagnosis
2020-07-15 07:09:07 PM maryneals48 yes the cookbook
2020-07-15 07:09:12 PM clcroom Our department is non-medicinal outreach.
2020-07-15 07:09:16 PM suzannemmartin where do you find it
2020-07-15 07:09:19 PM davislsc sometimes a resident will have more that one

2020-07-15 07:09:42 PM melh how many on here have used the diagnosis in their facility
2020-07-15 07:09:43 PM kayleeserafini most residents have more than one active diagnosis

2020-07-15 07:10:06 PM sierrafannin that explains it
2020-07-15 07:10:10 PM martinezbetty assisted living here
2020-07-15 07:10:14 PM erinm1 hello everyone, sorry im late
2020-07-15 07:10:16 PM lynnh where do u get the book
2020-07-15 07:10:18 PM melh hello
2020-07-15 07:10:21 PM sierrafannin I find that adult day is completely different
2020-07-15 07:10:28 PM lynnh I work in Skilled
2020-07-15 07:10:29 PM melh yes it is
2020-07-15 07:10:32 PM davislsc if you have to do a lot of docutmenting you will have to learn how to use MDS 3.0
2020-07-15 07:10:35 PM suzannemmartin I use it every day to chart, so how do you use it
2020-07-15 07:10:44 PM melh good skills to learn
2020-07-15 07:10:49 PM clcroom I work SNF. Have a difficutl time figuring it out.
2020-07-15 07:10:53 PM stargie69 yes,
2020-07-15 07:11:00 PM davislsc Now MDS 3.0 does everthing for you since going digital
2020-07-15 07:11:00 PM sierrafannin We have activity participation charts i have to fill out and attend care plan meetings
2020-07-15 07:11:14 PM tashawade I work in skilled nursing facility the MDS 3.0 is very beneficial. It helps to establish what the residents prefers and how important things are to them
2020-07-15 07:11:18 PM sierrafannin We are still paper and pencil
2020-07-15 07:11:18 PM melh like anything I am sure just time using it and you would get the hang of it
2020-07-15 07:11:23 PM maryneals48 same here sierrafannin
2020-07-15 07:11:29 PM suzannemmartin but where does the cookbook get accessed
2020-07-15 07:11:32 PM lynnh same here
2020-07-15 07:11:38 PM davislsc now all you have to is enter the ICD code and a trigger will pop up
2020-07-15 07:11:42 PM sierrafannin for the most part. we have a database with information mostly for nursing though
2020-07-15 07:11:48 PM erinm1 yes my department does the F page for all new residents
2020-07-15 07:11:52 PM diovanneb yes, ours too
2020-07-15 07:11:57 PM davislsc once you know all the triggers the cookbook comes into play
2020-07-15 07:11:58 PM melh yes our nurses handle that
2020-07-15 07:12:01 PM suzannemmartin where do you enter the code
2020-07-15 07:12:01 PM kayleeserafini usually with activities we just document how the resident is doing in their quarterly or annual but we have access to all the diagnosis from nurses and everyone else
2020-07-15 07:12:05 PM sierrafannin That would be amazing, because i chart on 40-60 people a day
2020-07-15 07:12:07 PM lynnh We dont see anything cause our Director hinds everything and has her own room locked up
2020-07-15 07:12:10 PM martinezbetty hmmmm..will we need this in AL?
2020-07-15 07:12:11 PM davislsc there are three steps to the cookbook
2020-07-15 07:12:25 PM diovanneb thanks for the tip davisisc
2020-07-15 07:12:38 PM diovanneb yes, teach us
2020-07-15 07:12:39 PM sierrafannin everytime they participate in an activity and what level of participation they had
2020-07-15 07:12:49 PM davislsc one find the trigger
2020-07-15 07:12:57 PM davislsc two creat a goal
2020-07-15 07:13:09 PM sierrafannin So like Isolation
2020-07-15 07:13:13 PM clcroom I do the same, sierra, I also do progress notes, care plans, assessments, etc.
2020-07-15 07:13:13 PM melh who identifies the diagnosis
2020-07-15 07:13:23 PM sierrafannin goal- be more social?
2020-07-15 07:13:36 PM martinezbetty participate more
2020-07-15 07:13:36 PM kayleeserafini it's important to do the 7 day look back to help find the trigger for the active diagnosis if there is a trigger it should be listed
2020-07-15 07:13:38 PM lynnh whaat for it
2020-07-15 07:13:38 PM sierrafannin clroom- are you in adult day
2020-07-15 07:13:39 PM clcroom I believe nursing identifies the dagnosis.
2020-07-15 07:13:55 PM lynnh what is the third
2020-07-15 07:13:59 PM sierrafannin our participants get Dr referrals
2020-07-15 07:14:01 PM clcroom No, Skilled Nursing. Have not figured out the MDS.
2020-07-15 07:14:02 PM davislsc THe resisdent will tell you thier own diagnosis
2020-07-15 07:14:15 PM martinezbetty thats true
2020-07-15 07:14:23 PM lynnh Plus more
2020-07-15 07:14:27 PM melh Physicianb. Physician assistantc. Nurse practitionerd. Clinical nurse specialist
2020-07-15 07:14:28 PM davislsc like if they have hypertention or diabietic
2020-07-15 07:14:31 PM martinezbetty way more lol
2020-07-15 07:14:32 PM sierrafannin Clcroom- yes im not sure about that either
2020-07-15 07:14:32 PM kayleeserafini it's important to check the MDS 3.0 section to know what residents have what active diagnoses
2020-07-15 07:14:45 PM kayleeserafini not all residents can tell you their diagnoses
2020-07-15 07:14:58 PM diovanneb you r right
2020-07-15 07:14:58 PM clcroom I've gone through it. Tried to use it. Havne't figured out how it applies.
2020-07-15 07:14:58 PM kayleeserafini some residents can
2020-07-15 07:14:59 PM davislsc If you are using MDS3.0 you are working with a clinical team
2020-07-15 07:15:11 PM suzannemmartin so where do you put the code into to get a trigger
2020-07-15 07:15:12 PM maryneals48 Right
2020-07-15 07:15:17 PM davislsc You as activity dept are part of that team
2020-07-15 07:15:18 PM sierrafannin our name tags with icons related to their diagnoses helps our staff while still respecting them
2020-07-15 07:15:23 PM martinezbetty are we supposed to have access to the medical records of the residents?
2020-07-15 07:15:36 PM diovanneb yes we can
2020-07-15 07:15:46 PM melh and as a team its good to know their background
2020-07-15 07:15:47 PM sierrafannin I have full access to mine
2020-07-15 07:15:54 PM kayleeserafini if you have access to MDS notes you have access to medical records
2020-07-15 07:16:02 PM janinereyesad It helps us create a plan for the res
2020-07-15 07:16:03 PM tashawade I have access to all resident's charts
2020-07-15 07:16:06 PM sierrafannin we also get intake sheets for each person
2020-07-15 07:16:07 PM martinezbetty I get a face sheet but thats it
2020-07-15 07:16:16 PM melh and to understand their needs
2020-07-15 07:16:21 PM clcroom We have access to the medical records.
2020-07-15 07:16:23 PM maryneals48 I have access to all mines
2020-07-15 07:16:31 PM martinezbetty i see the importance
2020-07-15 07:16:45 PM suzannemmartin I am still not sure how this works
2020-07-15 07:16:49 PM sierrafannin It is very important to have access to the charts
2020-07-15 07:16:50 PM erinm1 we are paperless, everything is on the computer
2020-07-15 07:17:16 PM sierrafannin for the most part everything that is in their charts at our facility is on our Hub network
2020-07-15 07:17:26 PM melh you should be able to read on the computer their diagnosis
2020-07-15 07:17:28 PM janinereyesad We have paper charts still
2020-07-15 07:17:34 PM tashawade same here sierafannin
2020-07-15 07:17:37 PM stargie69 we are too
2020-07-15 07:17:45 PM lynnh we have paper charts too
2020-07-15 07:17:50 PM clcroom We still chart on paper. Only Actvities. All others ar eon computer.
2020-07-15 07:17:55 PM kayleeserafini What would be a good way to help someone with an active vision diagnosis to participate in activities?
2020-07-15 07:17:58 PM erinm1 yes we have easy access to all residents information
2020-07-15 07:17:59 PM melh e have paper charts and on the computer
2020-07-15 07:18:03 PM melh we**
2020-07-15 07:18:08 PM maryneals48 We have both
2020-07-15 07:18:15 PM davislsc So a goal is for instance is when a residents finds loss of interest of activity based on a diagnosis like embarissment or depression your job as ACT dept is to find proprer activities to work with this resident
2020-07-15 07:18:32 PM sierrafannin agreed
2020-07-15 07:18:35 PM melh yes
2020-07-15 07:18:42 PM lynnh true
2020-07-15 07:18:44 PM martinezbetty right
2020-07-15 07:18:45 PM maryneals48 right
2020-07-15 07:18:46 PM tashawade yes
2020-07-15 07:18:48 PM suzannemmartin so how does MDS cook it up
2020-07-15 07:18:50 PM kayleeserafini Yes something that interests them to help them feel and stay engaged
2020-07-15 07:18:50 PM stargie69 yes, true
2020-07-15 07:18:52 PM erinm1 agreed
2020-07-15 07:18:52 PM clcroom Yes. However, the medical does the diagnosis.
2020-07-15 07:19:08 PM clcroom I observe and then modify if needed.
2020-07-15 07:19:13 PM melh think outside the box to help them be the most successful
2020-07-15 07:19:22 PM sierrafannin we deal with all different levels of cognative abilities thats why i like to utilize small groups
2020-07-15 07:19:33 PM martinezbetty good one
2020-07-15 07:19:38 PM sierrafannin more indiviualized and can be more inviting
2020-07-15 07:19:41 PM maryneals48 great
2020-07-15 07:19:59 PM clcroom We're not allowed groups. Except the memory care unit.
2020-07-15 07:19:59 PM kayleeserafini 1:1 are good for someone with depression who doesn't like to leave the comforts of their room
2020-07-15 07:20:03 PM melh and smaller groups usually they ar there cause of the same interest
2020-07-15 07:20:04 PM maryneals48 small groups are awesome
2020-07-15 07:20:05 PM diovanneb yes, survey states we need to be more individualized
2020-07-15 07:20:09 PM martinezbetty it helps a lot if they have a friend there a buddy
2020-07-15 07:20:12 PM mclaughlinjenny enter the chat room
2020-07-15 07:20:13 PM tashawade we are not doing any form of groups now. It is all one to one
2020-07-15 07:20:23 PM suzannemmartin I thought if a resident gets embarrased I would do a one on one activity and talk with them and encourage them to try something else they would like to do
2020-07-15 07:20:28 PM davislsc Well medical doesn't give the diaganosis, a doctor does but we still have to listen to the resident to give them a sense of dignity
2020-07-15 07:20:29 PM martinezbetty yes in-room activities
2020-07-15 07:20:32 PM erinm1 thats why interviews are so important
2020-07-15 07:20:32 PM sierrafannin our higher functioning participants sometimes find it hard to participate with lower functioning and this also helps with that
2020-07-15 07:20:33 PM clcroom Friendly visits
2020-07-15 07:20:47 PM sierrafannin yes assessments are key
2020-07-15 07:20:48 PM melh we have three wings smaller groups and one on one
2020-07-15 07:20:55 PM suzannemmartin so I still don't understand the cookbook
2020-07-15 07:20:55 PM maryneals48 yes only in room visits for now
2020-07-15 07:20:58 PM melh yes they paint a picture
2020-07-15 07:21:06 PM sierrafannin they sure do
2020-07-15 07:21:07 PM melh using the assessment
2020-07-15 07:21:17 PM martinezbetty show us how to cook DAV
2020-07-15 07:21:18 PM diovanneb hahaha...Sue...me too!
2020-07-15 07:21:23 PM sierrafannin our nurses also perform MOCA tests on them
2020-07-15 07:21:25 PM kayleeserafini Everyone involved in the residents care/ well being need to be involved in helping with resident have the best quality of life with their active diagnosis
2020-07-15 07:21:26 PM davislsc You will probably know the diagnosis a head of time but you will have to pretend like you don't when you meet a resident for the 1st time
2020-07-15 07:21:37 PM diovanneb yes, I do that
2020-07-15 07:21:38 PM clcroom I'm looking at it and see not codes, just what I would find on my part of the care plan at work.
2020-07-15 07:21:42 PM mclaughlinjenny i do that
2020-07-15 07:21:43 PM melh you find out what they care capable of what they understand
2020-07-15 07:21:44 PM tashawade I agree
2020-07-15 07:21:53 PM diovanneb I allow the resident to express themselves
2020-07-15 07:21:54 PM suzannemmartin yes I agree with that
2020-07-15 07:21:55 PM maryneals48 agree
2020-07-15 07:22:13 PM sierrafannin agreed, lets you challenge them in a postive way too
2020-07-15 07:22:16 PM clcroom I try to have a conversation with them instead of just firing questions.
2020-07-15 07:22:28 PM suzannemmartin yes conversations work better
2020-07-15 07:22:38 PM clcroom More info is volunteered comfortably.
2020-07-15 07:22:42 PM sierrafannin yes i make a conversation out of my assement too
2020-07-15 07:22:45 PM erinm1 conversation is a great way to get to know the resident
2020-07-15 07:22:45 PM diovanneb gain trust
2020-07-15 07:22:51 PM martinezbetty yessss
2020-07-15 07:22:52 PM davislsc after a goal has been set up like more mental activity, physical, spiritual, the cookbook gives you activies on how to excute those goals
2020-07-15 07:22:52 PM maryneals48 agree with the conversation first
2020-07-15 07:22:55 PM tashawade I normally do a little of both. Have a conversation and ask the questions
2020-07-15 07:22:57 PM mclaughlinjenny conversation is better get to know them better
2020-07-15 07:22:58 PM melh exactly, we had a gentleman who came to us told one things that when he arrived and we learned werent true at all so you need to not take what is given as true
2020-07-15 07:23:09 PM suzannemmartin but where is the cookbook
2020-07-15 07:23:11 PM melh and you get to know them
2020-07-15 07:23:17 PM sierrafannin oh the stories i have heard!! lol
2020-07-15 07:23:24 PM kayleeserafini involve them in activities that peek their interest
2020-07-15 07:23:25 PM martinezbetty lol yup
2020-07-15 07:23:25 PM melh lol
2020-07-15 07:23:28 PM tashawade lol
2020-07-15 07:23:30 PM clcroom You have to buy the cookbook, or see if your company will.
2020-07-15 07:23:36 PM davislsc again you will only have to use MDS 3.0 in a skilled nursing setting
2020-07-15 07:23:46 PM diovanneb i got it! cookbook...you mean, if the goal is spritual...the list of activities are writen there
2020-07-15 07:23:46 PM davislsc not al

2020-07-15 07:24:06 PM clcroom I work in a skilled nursing setting......still haven't used it.
2020-07-15 07:24:06 PM suzannemmartin I have to bring it home to work on
2020-07-15 07:24:23 PM melh i work in an ICF IID Nursing and I dont use it
2020-07-15 07:24:24 PM sierrafannin yes i too bring home work

2020-07-15 07:24:35 PM clcroom O bought my cookbook.
2020-07-15 07:24:37 PM sierrafannin prep materials
2020-07-15 07:24:39 PM suzannemmartin so the cookbook is not in regular MDS, you still have to buy the cookbook?
2020-07-15 07:24:42 PM tashawade really? clcroom? I have all the questions memorized by now
2020-07-15 07:24:50 PM mclaughlinjenny whats the cook book
2020-07-15 07:24:55 PM davislsc Just depends on how your state wants you to operate your facility
2020-07-15 07:25:00 PM mclaughlinjenny im late to the convo sorry
2020-07-15 07:25:02 PM kayleeserafini it's not an actual cook book
2020-07-15 07:25:16 PM melh hello
2020-07-15 07:25:21 PM davislsc our sate in tx requirres the use of mds3.0 im skilled nursing

2020-07-15 07:25:34 PM sierrafannin can someone actually explain the "cook book" for those of us unfamiliar

2020-07-15 07:25:47 PM suzannemmartin hold on he is trying
2020-07-15 07:25:54 PM sierrafannin lol
2020-07-15 07:25:55 PM kayleeserafini SNF use MDS 3.0 because mostly everyone is long term and have many active diagnoses at one time
2020-07-15 07:26:22 PM sierrafannin i know we have billing codes...
2020-07-15 07:26:24 PM melh yes many
2020-07-15 07:26:26 PM kayleeserafini Everyone with access to MDS in the SNF can access MDS 3.0
2020-07-15 07:26:27 PM clcroom I'm in FL and have never been asked about it. Thought it would be a good resource to have.
2020-07-15 07:26:34 PM sierrafannin would this relate to that in anyway
2020-07-15 07:26:41 PM suzannemmartin just because someone has a diagnosis of cancer, it doesn't mean that whatever the trigger is will work for every resident, this is what I don't understand
2020-07-15 07:26:46 PM kayleeserafini each diagnosis has it's own unique code
2020-07-15 07:26:48 PM melh yes it is another tool under your bet
2020-07-15 07:26:51 PM melh belt**
2020-07-15 07:26:52 PM cmckinny enter the chat room
2020-07-15 07:27:04 PM clcroom Mine has no trigger codes.
2020-07-15 07:27:05 PM davislsc I just want you guys to know that MDS 3.0 looks really complicated but it is now digital and other tools are there should you become part of a care plan team
2020-07-15 07:27:06 PM martinezbetty stock up on tools
2020-07-15 07:27:10 PM sierrafannin see i dont do any part of that
2020-07-15 07:27:12 PM kayleeserafini each MDS 3.0 is specifically tailored to each resident
2020-07-15 07:27:19 PM kayleeserafini like a care plan
2020-07-15 07:27:23 PM davislsc has anyone been part of a care plan team?
2020-07-15 07:27:28 PM clcroom I am part of a care plan team.
2020-07-15 07:27:28 PM sierrafannin yes
2020-07-15 07:27:31 PM kayleeserafini yes

2020-07-15 07:27:51 PM tashawade no not yet
2020-07-15 07:28:02 PM kayleeserafini All women's alzheimers unit

2020-07-15 07:28:04 PM mclaughlinjenny just starting so i dont have mjuch input for them yet
2020-07-15 07:28:07 PM davislsc ok chances are after you become certified you will have to learn how to use MDS3.0
2020-07-15 07:28:09 PM suzannemmartin you learn something new every day with this class
2020-07-15 07:28:13 PM melh Assessment consists of what two-stepstep process
2020-07-15 07:28:14 PM tashawade left the chat room
2020-07-15 07:28:18 PM stargie69 yes
2020-07-15 07:28:21 PM tashawade enter the chat room
2020-07-15 07:28:28 PM davislsc The cookbook is a great tool to have
2020-07-15 07:28:36 PM sierrafannin I find it hard to relate being in adult day
2020-07-15 07:28:40 PM clcroom I'm ADCP
2020-07-15 07:28:53 PM davislsc if I can find where my boss got hers I will post a link in the student forum
2020-07-15 07:28:54 PM sierrafannin even in the chats on tuesdays
2020-07-15 07:29:09 PM mclaughlinjenny ok great was just going to ask where to get it
2020-07-15 07:29:12 PM suzannemmartin I usually burn everything I cook, lol, hopefully I can figure out what this cookbook is and if we have it
2020-07-15 07:29:21 PM kayleeserafini if you're not a nurse you really don't fill out anything in MDS 3.0 you just have to know what the codes mean and how to adapt around active diagnosis and help assist the residents
2020-07-15 07:29:25 PM sierrafannin (lol)
2020-07-15 07:29:29 PM davislsc Like the Ftags MDS3.0 will change and its a big hassel
2020-07-15 07:29:44 PM diovanneb too much work
2020-07-15 07:29:47 PM melh 1: Identify any diagnosesapplicable to the resident.Step 2: Determine if thediagnosises are active.
2020-07-15 07:29:48 PM davislsc Kayle is right
2020-07-15 07:30:23 PM sierrafannin so how does it pertain to activities?
2020-07-15 07:30:39 PM davislsc it's better to know the codes to work with other care plan team directors
2020-07-15 07:30:43 PM martinezbetty its better to know too much than not enough
2020-07-15 07:30:55 PM melh it is good in the sense you can learn about the resident and what you may need to do to adapt
2020-07-15 07:30:57 PM maryneals48 agree
2020-07-15 07:31:00 PM clcroom I do Section F for all residents and have care plan meetings Mon-Thurs. Long term, memory care and rehab.
2020-07-15 07:31:03 PM suzannemmartin I had a resident that had cancer and a stroke, she attended almost all activities and we had to help her with everything since she could only use one hand, but she really liked being with everyone and didn't care if she couldn't do the activity or not
2020-07-15 07:31:13 PM kayleeserafini So if a person has diabetes and their sugar levels are super high one day you might avoid bringing them to a food oriented activity or offer sugar free food.
